Membership Plan

At Sleek Dental we offer a comprehensive dental membership plan that gives you access to the personalized preventive care you need. Our affordable plan includes the following:

  • 2 Doctors Exams
  • 2 Emergency exams
  • 2 hygiene cleanings
  • 2 fluoride tooth desensitizing treatments
  • 2 oral cancer screenings
  • X-rays/Pano/3D Scan
  • 50% Gum Disease Treatment
  • 20% off services such as but not limited to dental sealants, fillings, or extractions, implants, root canals
  • (Check with office for further details)

Our membership plan fees are as follows:

  • Child: $199/yr
  • Adult: $99 Initiation Fee, $349/yr

Program Exclusions

  • In conjunction with another dental plan, dental insurance, or financing.
  • For treatment which, in the sole opinion of our doctors, lies outside the realm of their capacity.
  • For referral to specialists and in-house specialists
  • For hospitalizations or hospital charges of any kind.
  • For cost of dental care which are covered under automobile medical.
  • For services of injuries covered under automobile medical.
  • This plan is only honored at Sleek Dental and can not be used at any other office.

Program Guidelines

  • Patients portion of the bill is due the day of the service
  • There will be a $99 reinstatement fee if your plan lapses
  • Cannot be used with another plan without approval
  • No refunds of membership payment
  • NON REFUNDABLE
